Option 1:
You can use the link below to print out form I-9 online. As the employee you will complete and sign section 1. You will then bring your signed form I-9 and identification documents with you to meet in person with your union representative or steward who will complete Section 2. Once both sections are completed the finished document can be emailed to UTP.
IMPORTANT NOTE: ALL DATES MUST BE IN MM/DD/YYYY FORMAT.
Option 2:
If you are unable to complete your form I-9 in person with a union representative, you can use the link below to complete our employee contact information form. UTP will then use the information you provide to send you an unique email link to the E-Verify+ website where you can complete your form I-9 online and upload your identification documents directly through E-Verify+.
Once completed you will receive a follow-up email from UTP to schedule a zoom meeting to remotely verify your employment authorization documents. You will need to have the same documents that you provided to E-Verify+ with you at the time of the zoom call.
